**About the Role**
Our Social Worker liaises with various healthcare and social services providers to ensure that our clients are receiving timely and appropriate home and community based care. The role is responsible for ensuring that quality and person-centered clinical care is provided to NDIS participants to help minimise preventable hospital admissions. Social Workers work closely with the stakeholders and guardians of participants to support them to access the most appropriate services for their needs, and they seek to provide a holistic support base both to clients and to the support staff who work with clients.
**Key Duties Include**:
- Determining the nature of required supports for NDIS participants via initial psychosocial assessments.
- Monitoring the progress of clients through periodic follow-up meetings.
- Undertaking care planning activities and ensuring that services are being managed effectively.
- Maintaining accurate and thorough records and remaining organised for evaluation/follow-ups.
- Prioritising client confidentiality, security and privacy.
- Obtaining assistance for clients by referring them to appropriate services or arranging appointments with other agencies.
**This role is based at our central office in Loganholme. The role will be hired on a permanent, full time basis.**
**Skills & Experience**
**Key Requirements Include**:
- Possess a tertiary qualification in Social Work.
- Member of the Australian Association of Social Workers.
- Possess or willing to obtain an NDIS Worker Screening Check.
- Possess or willing to obtain a Blue Card.
- A valid QLD licence and a reliable vehicle.
