**Position Objective**
The Community Care Team Leader is responsible for providing operational leadership,
management and coordination of the Community Care functions including Case Coordination,
Support Work, Recreation and Transport.
As a representative of Council, demonstrated professionalism and a commitment to a high level of

**Key Responsibilities**
**_Leadership & Service Management_**
- Manage and oversee operations of Community Care program including staffing, financial
management, rostering, purchasing, record management and reporting requirements
- Work collaboratively with the Community Care team to ensure best practice is being
integrated across services and providing best care solutions to clients
- Monitor, review and report on Community Care operational outputs, service standards,
compliance with Council policy and procedures, and staff development requirements to
Council and funding bodies as required
- Oversee a fair and equitable roster system aligned to client services for Flinders Shire
- Lead the Community Care team and engage as an active team member and role model,
ensuring communication is maintained and team roadblocks are removed where possible
- Provide constructive feedback and manage performance of team including undertaking
performance reviews within Council’s performance cycle
- Review and report concern of health and safety for staff, volunteers and clients during
employment activities and actively participate in removing risks
- Manage and coordinate quality and safety audit requirements and self-assessment as
ongoing commitment to continuous quality improvement
- Develop, review and implement policies, work practices and procedures for the service
- Proactively source funding options to align Community Care objectives and Council’s
strategic directions
**_Care Management & Coordination _**
- Manage and oversee the client journey from initial contact to ensure a consistent and high
- quality experience
- Working with the Assistant Coordinator, undertake all requirements care coordination and
management to support clients care needs, including development of care plans, client
budgets (for Home Care Package clients only), purchases of equipment and modifications,
and referral to clinical support services as required
- Oversee and hold meetings with clients' families to ensure clear communication and
achievement of objectives
- Oversee maintenance of client records and documentation in line with accreditation
standards
- Develop and maintain operational service systems and processes to support effective
service management
with regulations, legislation, and funding agency agreements
- Coordinate and promote Council’s aged care independent living centre and accommodation
options including rental agreements and facility bookings
- Develop, review and deliver in-house training for team in line with accreditation standards
- Research and recommend opportunities for service development and expansion in line with
client and community needs
