**Who We Need**

**Key Responsibilities**
- Act as the first point of contact for new and existing clients, providing **clear and compassionate guidance** throughout the onboarding process.
- Manage client **intake, assessment, and documentation**, ensuring compliance with aged care regulations.
- Coordinate with the **HCP Care Team, Reception, and other stakeholders** to facilitate smooth service delivery.
- Address client inquiries, concerns, and complaints **professionally and efficiently**.
- Maintain accurate client records and databases in accordance with **privacy laws and industry standards**.
- Educate clients and families on their **Home Care Package (HCP) funding and service options**.
- Assist with reports, administrative tasks, and projects as required.
- Work collaboratively with internal teams to enhance service quality and client satisfaction.

**To Be Successful in This Role, You Will Have**:

- Experience in **client liaison, intake, or case management** within aged care, healthcare, or community services.
- A **genuine passion** for supporting older Australians.
- Strong **communication, problem-solving, and organizational skills**.
- Ability to engage with people from **diverse backgrounds and cultures**.
- Proficiency in **MS Word, Excel, and client management systems**.
- Knowledge of **Home Care Packages (HCP) and My Aged Care processes** (highly regarded).
- A **valid Driver’s License** and **full rights to work in Australia**.
- **National Police Check** (or willingness to obtain one).
