**About the role
Temporary full time opportunity to deliver high quality, evidence based social work services, including care co-ordination and case management as a member of the multidisciplinary team to consumers of the Older Persons Mental Health Service with severe and complex mental health disorders, their carers and relevant stakeholders.
About you
Deliver competent level social work services, including care co-ordination and case management to older persons with complex mental health treatment needs based on valid and reliable evidence, in accordance with the Australian Association of Social Workers (AASW) Code of Ethics and AASW Professional Practice Standards and mental health standards to ensure culturally appropriate consumer focused outcomes.

Participate in the identification, development, implementation and evaluation of quality and service improvement / research activities within the Mental Health Service that continually improve consumer care and enhance the delivery of services within a multidisciplinary team and the service.
