**Your Role**:

- **Relationship Building**: Forge strong connections with clients and care workers, ensuring seamless communication and exceptional service.
- **Client Management**: Oversee a caseload of up to 30 clients with various conditions, coordinating care plans, managing budgets, and promoting independence.
- **Home Visits**: Introduce clients to our compassionate care workers and provide ongoing support through home visits.
- **Administrative Excellence**: Utilise your computer skills for efficient record-keeping and communication.
- **Responsive Support**: Answer calls from clients and care workers, ensuring their needs are met promptly.
- **Compliance**: Conduct care plan reviews and maintain compliance documentation.

**About You**:

- **Experience**: Minimum of 12+ months in the Aged and Disability Sector, or in a similar interpersonal role.
- **Qualifications**: Valid QLD Driver’s License, First Aid & CPR Certificate, COVID-19 Vaccinated, Yellow Card/NDIS Worker Screening Clearance (Paid), Federal Police Check.
- **Skills**: Strong computer literacy and excellent interpersonal skills.

**Schedule**:

- 8 hour shift
- Monday to Friday
- On call
- Overtime
