**About the role
Permanent full time opportunity to deliver Social Work services for adolescents and young adults (aged 14-25 years) experiencing moderate to severe substance use disorder and/or complicated by other mental health concerns, and their families/significant others, to achieve maximum benefits from health care services in accordance with organisational goals.
About you
As a member of the multidisciplinary team, deliver contemporary competent level Social Work services based on valid and reliable evidence, are in accordance with the Australian Association of Social Workers (AASW) Code of Ethics and professional practice standards and performed with a high degree of independence.
Actively participate in the multidisciplinary team by providing routine discipline specific clinical formulation and advice, as well as contribute to the development of individualised treatment plans including discharge planning and service provision.
Participate in the development and implementation of practices, procedures, protocols and other quality and service improvement and research activities for adolescents and young adults within Metro North Mental Health - Alcohol and Drug Service (MNMH-ADS) to continually improve consumer care.
