We are looking at hiring Support Coordinators with their own case load to join our growing team for the South East and Northern suburbs of Melbourne.

**The Role**
- A Support Coordinator is responsible for supporting the National Disability Insurance Scheme (NDIS) Participants to activate, interpret and implement their NDIS Plans.
- As a Support Coordinator, you will work with scheme participants between the ages of 0 - 65 with varying complexity levels.
- You will coordinate service providers and supports, liaise with government agencies, obtain quotes for services, organise service agreements and monitor Participants plans.
- You will also provide regular Support Coordination reports to the NDIA for each Participant you support.

- Excellent verbal, written communication skills, efficient computer skills
- Excellent customer service skills and genuine care and attention to our clients and participants.
- Good time management and organisational skills.
- Ability to exceed KPI's and multitask.
- Be able to work in a team but also work well independently
- Be able to work in line with the organisations vision and values.
- Have a case load of clients

**Qualifications & Experience**:

- Minimum Certificate IV in Disability or related field
- Demonstrated knowledge and experience of NDIS
- Current First Aid and CPR Certificate (or willing to obtain)
- Victorian Driver's license and vehicle
- NDIS Worker Screening check (or willing to obtain)
- Working with Children Check

**Tasks & Responsibilities**:

- Liaising and building rapport with key stakeholders such as: NDIS Providers and Agents, community venues, community partners, along with other revenue streams or allied health care teams.
- Monitor plan outcomes and expenditure to ensure the services remain within the plan budget for participants.
- Independently manage a varied caseload of participants in the community ensuring quality of care and meeting client objectives and requirements.
- Conducting initial assessments
- Researching and referring participants to relevant services when required with a focus on Recovery.
- Ensure participants are working towards achieving their goals and being able to clearly document progress in reports

**Salary**: From $66,000.00 per year

Schedule:

- Monday to Friday
