Established since 2009, My Home Living Care (MHLC), is a registered NDIS provider, specialising in home and community care support.

We are dedicated to providing quality care and support services that are person-centred, culturally appropriate, and responsive to our customer’s home and community needs.

**About the Role**:
As a Support Coordinator you are a central point of contact for NDIS participants, their family members, support workers and all other stakeholders. Your main responsibility is to ensure delivery of quality services, budget management and working with the rostering team.

**Responsibilities**:

- Coordinate clients’ referrals, assessments, care plannings and reviews
- Actively promote core support services to prospective NDIS Participants
- Create and continuously develop relationships among Participants, their family members, Support Workers and all other stakeholders
- Monitor and evaluate service delivery, including regular review of client files and services.
- Forecast and monitor individual participant’s budget to ensure appropriate support delivery within the budget
- Provide accurate case notes and progress reports that reflect forward planning in line with the participant's NDIS plan
- Lead the Support Worker teams to provide responsive and person-centered services
- Coordinate Support Workers roster for new participants and proactively resolve rostering issues

**Requirements**:

- Knowledge of the NDIS Rules and the Quality and Safeguards Framework
- Work experience in the disability industry
- Positive, patient and a passion for helping others
- A professional level of administrative and business communication skills, including case noting and report writing
- Strong interpersonal skills and high levels of resilience
- Excellent time management and organisational skills
- Own car and driver's licence

**Salary**: From $65,000.00 per year

Schedule:

- Day shift
- Monday to Friday

Supplemental pay types:

- Performance bonus

**Education**:

- Bachelor Degree (preferred)

Licence/Certification:

- Drivers Licence and Roadworthy Vehicle (preferred)
