Flourish Holistic Services PTY LTD is looking for an Allied Health Assistant for our Tarneit office.

**To apply**:
Full-time position ($27-$30/hour) depending on your experience level.

Working hours: Monday - Friday 9.30 am - 5.30 pm, and weekends (depending on the participant’s needs)

Opportunity to work in the community setting as per referral requirements.

Have a person-centered approach to clients and stakeholders

Recovery approach to participants and support plan

Implementation of a safe environment model in the delivery of care.

The purpose of this role is to use evidence-based practices to prevent, diagnose and treat various conditions and illnesses, and provide specialized support to suit an individual's needs.

**Responsibilities**:
Work with other occupational therapy and physiotherapy allied health professionals to implement a multidisciplinary care plan and perform sessions under their guidance.

Perform community outreach, home visits, and clinical-based, and telehealth sessions.

Educate the participants and their families to optimize their ability to live healthy lives within the community.

Liaise with the clinical manager to ensure effective communication.

Communicating with participants’ families, stakeholders, and referrals

Provide recommendations as per the need of the participants.

Write progress notes and reports daily

Assess participants individually and prepare customized charts, exercise plans, or activities accordingly.

Travel to the client's house for the sessions.

**Requirements**:

- Hold a certificate III or IV in Allied Health Assistance
- Australian driver’s license (preferred)
- Current National Police Check (required)
- Valid Working with Children’s Check (required)
- NDIS Worker Screening Check (required)
- CPR Certification (required)
- First Aid Certification (required)
- At least 1 year experience in Allied Health (preferred)

**Salary**: $27.00 - $30.00 per year

Ability to commute/relocate:

- Tarneit, VIC 3029: Reliably commute or planning to relocate before starting work (required)

Licence/Certification:

- First Aid Certification (preferred)
- Working with Children Check (preferred)
- NDIS Worker Screening Check (preferred)
- National Police Certificate (preferred)
- Driver Licence (preferred)

Work Authorisation:

- Australia (preferred)
