As a Case Manager, reporting to the Operations Manager, your role is crucial in enhancing client management quality and maximizing care outcomes. You will manage the Home Care Packages of assigned clients, ensuring their health and wellbeing needs are met through assessment, planning, implementation, monitoring, and evaluation.

About the role:
Key responsibilities:

- Build positive, trusting relationships with clients, customers, allied health professionals, and care workers.
- Collaborate with the operations team to support clients' specific needs and goals, fostering independence, wellbeing, and peace of mind.
- Utilize best clinical case management practices to align care plans with client needs and deliver effective services.
- Monitor client progress and update case plans as needed.
- Coordinate with healthcare providers for continuity of care.
- Facilitate communication between clients, families, and healthcare providers for seamless care.
- Advocate for clients, ensuring they receive necessary services and support.
- Assist clients in navigating the healthcare system and accessing community resources.
- Monitor clients' utilization of home care package funding, ensuring effective and efficient use.

About you:

- Certificate III/IV in a related discipline.
- Demonstrated experience in a community/nursing role.
- Knowledge of the Home Care Packages system.
- Mandarin/Cantonese speaking preferred.
