Then you could be the perfect Connections Transitions Coordinator (HM1) to join the professionals at Justice Health and Forensic Mental Health Network. Specifically, we’re looking for applicants with a degree in a relevant field, or equivalent work experience, or a combination of study and work experience as well as Current NSW Driver's licence (Class 1A or C). Someone with demonstrated experience in managing continuity of care for clients with chronic and complex health needs. Importantly, if you are an overseas resident with a temporary visa, please apply. This position will be open to you if no suitable Australian permanent resident or citizen is identified.

Here at Justice Health & Forensic Mental Health Network, this project aims to improve continuity of care for patients of the correctional centres with Drug and Alcohol problems, who are being released to the community. The Connections Program works within an assertive linkage model that aims to link patients with relevant health and welfare service providers post release. The Connections Program team utilises a broad array of contacts, both in the correctional and community environments to assertively link patients into services appropriate to their individual needs. The role and responsibilities are to be carried out in a manner that is consistent with Network delegations, policies, procedures and operations systems, and in line with both the NSW Health and Network Codes of Conduct.

Justice Health and Forensic Mental Health Network is a Statutory Health Corporation established under the Health Services Act (NSW) 1997. We are part of the broader health system reporting to the Minister for Health through the Network Board and the Secretary, NSW Health.

- Demonstrated ability to work both independently and as part of a multi-disciplinary team and meet tight deadlines.
- Demonstrated good level of conflict resolution and negotiation skills and proven networking and excellent interpersonal skills.
- Must have a current drivers license and willingness to travel
