**The Salary range for this position is $ 32.77 to $ 35.55 per hour, plus reimbursement of $0.96 per KM. Hourly rate is based on experience.**

**Are you a compassionate and qualified Home Care Worker passionate about supporting older Australians to live independently and with dignity?**

Churches of Christ is seeking dedicated professionals to join our **Home Care team for the West Moreton and Scenic Rim Region.**

**What You’ll Do**

As a** Home Care Worker - Medication Support, you will:?**
- Provide personal care assistance, including showering, dressing, and toileting.
- Assist clients with medication management in accordance with care plans and organizational policies.
- Offer domestic support, including meal preparation and household tasks.
- Facilitate social engagement through transport and companionship.
- Monitor and report changes in clients' health to the Service Coordinator or Registered Nurse.
- Maintain accurate documentation and participate in care plan reviews.

**Qualifications and Requirements**
- Certificate III or IV in Individual Support (Ageing) or equivalent.
- Completion of HLTHPS006 - Assist Clients with Medication, HLTAAP001 - Recognise Healthy Body Systems, and CHCCCS031 - Provide Individualised Support.
- Current and up-to-date First Aid and CPR certifications.
- A minimum of 12 months' experience in a direct care role. (Desirable)
- A current driver's licence and a comprehensively insured vehicle that is suitable to be used for work purposes. **(Reimbursement of $0.96per KM is provided for use of own vehicle)**:

- Ability to read, write, and communicate effectively in English.
- Possess a Current National Police Certificate or willingness to obtain.
