**Renal Access Coordinator / Surgical Liaison Nurse - Nephrology**

**Our Journey**

This position creates an expansion of our existing **Renal Access Coordinator and Surgical liaison **role, a critical role that leads and drives the timely creation of access for Haemodialysis and Peritoneal Dialysis with a view to achieve optimal outcomes for all patients based on Nephrology guidelines and best practice standards. The role also advocates for ongoing access surveillance, timely interventions and a patient-centred approach.

The successful applicant will work with one of the most respected Dialysis Access Nurse CNCs in Australia and will be well supported by our leadership team to settle into this very important role in a phase of service expansion. This position will also support patients needs at the **Victorian Heart Hospital. This is a part-time.8EFT ongpoing position.**

**About Nephrology**

Monash Health is the quaternary provider of **Nephrology Services** in the south east corridor of metropolitan Melbourne and regional Victoria. Services include chronic kidney disease prevention and management, kidney and kidney-pancreas transplantation, a full suite of extracorporeal and peritoneal dialysis kidney replacement therapies, supportive care and comprehensive Nephrology research, education and training programs.

**Monash Health** provides Victoria’s largest dialysis service under an expanding hub and spoke model with the hub service located at Monash Health Clayton Campus. Our multidisciplinary teams provide holistic services that are designed and located to support a range of patient choices in a range of locations including inpatient, home and satellite dialysis.

Under the Department of Health Vic Renal Capability Framework 2019, Monash Health is a level 6 Hub provider with governance and support system responsibilities for its metropolitan, regional and rural dialysis partners.

**About You**

You have a passion for **Nephrology **and for ensuring that people with kidney failure are well prepared and supported in the lead up to and throughout the course of their dialysis journey. You will have experience in working with a wide range of stakeholders and be able to confidently and professionally advocate for patients to promote best outcomes. You will enjoy educating, supporting and promoting best practice in dialysis patient access management within our teams and have a strong drive for quality improvement.
