Newcomb Primary Care is located at the Newcomb Community Health centre in the heart of Newcomb. The centre is shared with numerous teams including Community Nursing, Dental Health, Mental Health, Renal Dialysis and Carer Gateway. Many programs, internal and external to Barwon Health visit and collaborate with our teams to provide holistic and life changing experiences for our clients.

**About the Role**

We are looking for a person that will thrive in contributing to and being part of the community based multidisciplinary Community Health team to support consumers and carers to achieve an optimum level of health independence through:

- Comprehensive, coordinated and integrated multidisciplinary services
- Provision of consumer centred care to community members from a broad range of backgrounds
- Proactively contributing to the development and delivery of high quality, cost effective community based dietetics service
- Contributing to meeting the program and Community Health team key performances (KPI

Essential: A qualification in Dietetics as defined by the Medical Scientist Award (HASUA 4)
Essential: A current Victorian driver’s Licence or equivalent
Essential: Accredited Practicing Dietitian with Dietitians Association of Australia
Essential: Experience across a range of Dietetics clinical settings
Essential: Ability to work collaboratively with consumers and service providers
Essential: Ability to manage a demanding work load and prioritise accordingly
Essential: Ability to work effectively as a member of a multidisciplinary team and contributes to a positive team en
