We are currently seeking an
**Accommodation and Supports Coordinator **to join our
**From Hospital to Home**
**(FH2H) **unit in South Perth, on a
**full-time** basis.

The FH2H unit is a purpose-built facility that houses and supports 12 participants at a time to build skills and capacity to live as independently as possible in the community. The FH2H team has dedicated support workers, administration, clinical & allied health support and unit management to allow you to build rapport and understand individual support needs to develop evidence to support funding requirements.

As our
**Accommodation and Supports Coordinator**, you will lead and mentor a team of Disability Support Workers to provide person-centred support to participants, all the while ensuring
**FH2H**
***is 100% compliant with State and Federal regulations, as well as the NDIS Practice Standards.

Your well-honed communication, assessment, and problem-solving skills are easily demonstrated when leading inter-disciplinary teams, as well as when making meaningful connections with participants with psychosocial disabilities.

Now... to ensure the team and participants of **FH2H** **get to work with you, it is essential you possess:

- Cert IV in Community Services/Disability/Mental Health
- National Police Clearance
- NDIS Worker Screening Check
- NDIS Worker Orientation Module _Quality, Safety and You_
- evidence of COVID-19 and Influenza vaccinations
- full working rights in Australia
