Our Bethanie Community team are seeking a **Clinical Case Manager** on a **Full Time** basis to support their clients in and around the **Metro South region.** The Case Manager is responsible for care coordination for community clients within a holistic reablement and wellness approach to enhance the lifestyle and living experiences of those residing at home. The Case Manager will participate in the implementation of a client directed model of care which focusses on empowerment of the client to maximise quality of life, promote healthy active ageing and facilitate community re-engagement.

This role is being offered on a **Permanent Full-Time basis**, **working 38 hours across Monday - Friday** and will require an individual who is willing and available to be on call after hours and on weekends
**About the Role**:
- Client and goal centred approach to care planning, incorporating regular and structured review processes to ensure currency and appropriateness of care plans is maintained
- Develops sustainable relationships with local networks and external agencies to strengthen access to social opportunities and lifestyle options across the service footprint
- Responsible for ensuring all client budgets are monitored and managed in line with approved package level and individual client requirements
- Be an effective member of the multi-disciplinary team, to ensure effective and holistic care is delivered to all clients.
- Provides reports, information and support as requested by the Regional Community Manager to a highly quality and in accordance to policy and procedures
**About You**:
- Holds current registration as an Allied Health professional or Registered Nurse with AHPRA - essential
- Experience managing a team of care providers
- Excellent customer service, negotiation, communication and support skills
- Excellent verbal and written communication skills and strong computer skills and experience with client database systems
- Valid driver’s licence and access to a reliable vehicle
**Desirable**:
- Knowledge of the Home Care Packages Programs, Commonwealth Home Support Program and Aged Care Standards
- Demonstrated experience in the Aged Care arena
