The Central Coast Local Health District is looking for an experienced and dedicated Manager to excel in our Transitional Aged Care Program (TACP) who will be responsible to the Aged and Complex Care Program Development Manager and manage the delivery of the Transitional Aged Care service as well as providing leadership to the multidisciplinary allied health team delivering the Central Coast Transitional Aged Care Program (TACP), as a part of Central Coast Local Health District.

**An Eligibility List (E-List) or talent pool may be created from this recruitment episode to assist in filling future permanent, temporary, or casual full-time and part-time positions.**

TACP provides short term care that seeks to optimise the functioning and independence of older people after a hospital stay. It is goal oriented, time-limited and therapy focused. Services are delivered in the community and include low intensity therapy, via occupational therapy, physiotherapy, and social work, nursing support or personal care. Overall aim is to support older people in returning to their home after a hospital stay rather than prematurely entering residential aged care.
The Central Coast TACP consists of a total of 128 packages made up of 119 permanent and 9 temporary packages - 18 residential packages in Woy Woy Transitional Care Unit (TCU) and 110 community-based packages (Transpac). The community-based packages are delivered in partnership with Central Coast service providers under a contracted service agreement arrangement.

**About You**
- Qualification in Nursing, Occupational Therapy, Physiotherapy, Speech Pathology, Social Work, Nutrition & Dietetics, and current professional registration &/or eligibility for membership of relevant professional association with a commitment to ongoing professional development.
- Demonstrated extensive experience in leading and managing a multi
- disciplinary team including experience with meeting the supervision needs of allied health, and nursing staff with varied levels of seniority and experience, performance management, and an understanding of current issues facing allied health professionals within CCLHD and broader contexts.
- Extensive experience and demonstrated ability in managing resources: human resources (performance management and encouraging others to build their professional skills) and financial (resource allocation and service planning).
- Demonstrated ability to communicate and negotiate effectively with a diverse range of key stakeholders including patients, carers, medical specialists, GPs, health care providers, and government and non-government community organisations.
