As an NDIS Support Coordinator, you will play a crucial role in helping our clients navigate the National Disability Insurance Scheme (NDIS) and access the support and services they require to thrive.

**Position Overview**: We are seeking a highly motivated and compassionate NDIS Support Coordinator to join our team. In this role, you will work closely with individuals with disabilities, their families, and support networks to develop and implement tailored support plans. You will be responsible for assisting clients in understanding their NDIS plans, identifying goals, connecting with service providers, and ensuring the delivery of high-quality support services.

**Responsibilities**:

- Conduct comprehensive assessments of clients' needs and goals.
- Collaborate with clients to develop individualised NDIS support plans.
- Assist clients in navigating the NDIS process, including plan reviews and appeals.
- Coordinate with service providers to ensure the delivery of appropriate support services.
- Advocate for clients' rights and needs within the NDIS framework.
- Provide ongoing support and guidance to clients and their families.
- Maintain accurate records and documentation in accordance with NDIS requirements.

**Requirements**:

- Bachelor's degree in social work, psychology, disability studies, or related field (or equivalent experience).
- Demonstrated understanding of the NDIS framework and processes.
- Excellent communication and interpersonal skills.
- Strong organisational and time management abilities.
- Ability to work independently and as part of a team.
- Compassionate and client-centered approach.
- Experience working with individuals with disabilities preferred.
