**Description**:
About the Role

Duties
- Accepting referrals from providers, individuals, carers seeking clinical evidence to complete the access process
- Collecting and collating all necessary evidence and completing the Access Request Form
- Assisting with submitting Access Request Form to the NDIA and acting as a contact person for NDIA queries
- Waiting list management intervention which includes: call back, cross sector referrals, gathering additional information etc
- Perform risk assessments (including referral to crisis intervention services, where necessary)
- Maintain accurate individual consumer files and data bases
- Facilitation of small group wellbeing programs (as required)

COVID-19 Requirements

Immunization history showing you have received two doses of a Covid 19 vaccine.

Required

Certificate III or higher qualifications in Health Care, Social Assistance, Community Services or Administration.
Knowledge, understanding and/or awareness of the NDIS space and the challenges the participants may face
Demonstrated ability to record accurate and consistent case notes.
Demonstrated ability to work with a minimum of direction and to effectively communicate with and work collaboratively as part of a multi-disciplinary team
Strong organisational skills and ability to work independently and/or as part of a team.
Previous experience in working with culturally diverse people.
Ability to communicate sensitivity and effectively with Aboriginal and Torres Strait Islander people.
Sound knowledge of Microsoft Suite Applications.
Current Drivers Licence (Qld).
Current National Police Clearance or ability to obtain.
Working with Children’s Card (Blue Card) or be eligible to apply.
Disability Worker Screening Card (Yellow Card) or be eligible to apply

Desirable

Previous experience of working with Aboriginal & Torres Strait Islander clients.
Previous experience in working with the National Disability Insurance Scheme (NDIS).
Lives or is knowledgeable in the Logan, Southern suburbs of Brisbane Community
