Care Coordinator

Utilise your skills and knowledge as a Care Coordinator for this Aged Care Respite House.
Requiring an immediate start, this role commences as a contract role with a view to permanency.
Reporting to the Respite House Manager, duties will include:
- Focusing on the service delivery as the primary contact for clients, families, advocates and stakeholders
- Managing onboarding of new clients, suitability assessments ensuring clients' needs and goals are met
- Coordinating day services & overnight respite for clients funded by CHSP, NDIS & HCP in line with the funder's guidelines and requirements
- Meeting with clients and their families to discuss clients goals and current needs, putting services in place and creating detailed & personalised care plans
- Managing a team of support workers
- Weekly rostering and scheduling
Key requirements:
- Demonstrate knowledge of CHSP and HCP
- Proven competence in creating and developing client care plans
- Excellent interpersonal and communication skills including the ability to manage stakeholder relationships
- Organisational skills and the ability to meet strict deadlines;
