Patient Care Specialist

Your primary role is to oversee and deliver high-quality patient care, ensuring that each individual receives tailored services.
- Conduct thorough assessments and develop comprehensive care plans, maintaining accurate documentation throughout the process.
- Respond promptly to health emergencies, identifying potential issues and managing complex care needs, including dementia, wound care, palliative care, and infection prevention.
- Administer medications as prescribed and ensure compliance with established quality standards.
- Lead and support nursing staff in delivering safe, compassionate care, promoting a positive work environment.
- Current registration with the relevant nursing authority.
- Degree or postgraduate qualification in Nursing.
- Proven clinical experience and knowledge of clinical care standards, preferably within aged care.
- Excellent interpersonal skills for engaging with patients, families, and colleagues, committed to person-centered care and growth.
- Valid police check and NDIS worker screening clearance (or ability to obtain).
