Job Summary:

We are seeking a highly skilled Clinical Care Specialist to join our Lung Transplant Team. As a key member of our multidisciplinary team, you will play a vital role in providing exceptional patient care and support.

The successful candidate will work closely with our Lung Transplant and Thoracic Surgery Teams to manage pre and post-transplant care for patients in our Oncology DREAM Program. This includes working with transplant nursing coordinators and assistants during pre-transplant work-up and evaluation, and post-transplant management and surveillance.

This is an outpatient position, with no nights, weekends, holidays, or on-call requirements. The Clinical Care Specialist will be responsible for their own clinic template during clinic days, and administrative time will be spent reviewing patient data and communicating with multidisciplinary team members.

The ideal candidate will have expertise in clinical care, education, and research, with a strong commitment to quality improvement. They will demonstrate exceptional communication skills, both written and verbal, and be able to work effectively in a fast-paced environment.

Key responsibilities include conducting comprehensive appraisals of patients' health status, providing direct clinical care, prescribing medications and ordering laboratory tests as needed, and evaluating patients to determine their need for further healthcare.

Required Skills and Qualifications:

  • A Master's degree from an accredited school of nursing or physician assistant program
  • Current licensure as a Registered Nurse in Illinois
  • Certification as an Advanced Practice Registered Nurse by a national certifying body
  • Privilege and credentialing through the NMH Medical Staff Office
  • BLS certification from the American Heart Association
