We're looking for someone with a strong sense of purpose and experience in the social services sector — a confident practitioner who leads with empathy and believes in people's ability to change and grow.

About You
  • We're seeking a skilled professional with a degree qualification in Social Work, Psychology, or a related field and demonstrated experience in case management within a homelessness or community services setting.
About The Role

As a Transitional Housing Case Manager, you'll be a vital part of a collaborative team working to empower individuals on their journey toward long-term, dignified housing. You'll walk alongside each individual, offering consistent, trauma-informed, person-centred support that builds resilience, independence, and hope.

  • Develop and implement strengths-based, solution-focused case plans in partnership with clients
  • Support transitions to long-term housing through warm referrals and coordinated outreach
  • Advocate across housing, health, legal, and community systems to meet client needs
  • Collaborate with internal teams and external partners to connect clients with relevant services
  • Maintain accurate records and contribute to service development and continuous improvement
