Job Title: Transitional Housing Specialist

We are seeking a compassionate professional to fill this critical role at our organization. As a Transitional Housing Specialist, you will work with individuals experiencing homelessness, providing support and guidance as they navigate complex life transitions.

The ideal candidate will have a strong sense of purpose, experience in the social services sector, and a commitment to social justice. You will be confident in your ability to lead with empathy and believe in people's capacity to change and grow.

About the Role:
  • Develop and implement tailored support plans that cater to clients' unique needs and circumstances.
  • Support clients in transitioning to long-term housing through targeted referrals and coordinated outreach efforts.
  • Act as an advocate across various systems, including housing, health, legal, and community services, to ensure clients receive comprehensive support.
  • Collaborate with internal teams and external partners to connect clients with relevant resources and services.
