Stable Housing Transition Coordinator

We are seeking a dedicated professional to play a crucial role in delivering comprehensive support packages to individuals who require assistance in sustaining tenancies or transitioning into stable housing.
The ideal candidate will possess a degree qualification relevant to community services and experience in case management, as well as the ability to navigate complex service systems effectively. The successful individual will focus on building strong relationships with participants who have experienced chronic homelessness, providing them with tailored support to achieve their goals.
A key aspect of this role will be working closely with participants to develop and implement individualized plans, conducting regular outreach appointments to ensure the best possible outcomes. Adaptability, effective communication, and a commitment to collaboration with various services will be essential skills for success in this position.
