We deliver care through Home Care Package and CHSP, ensuring continuity of care for our clients.
The Client Care Advisor ensures all care is delivered in alignment with client requirements and relevant internal and external standards.
You will be the primary point of contact, offering guidance, advocacy, and coordination of services to ensure our clients receive the assistance they need.
Key Responsibilities:- Case manage client care services, encompassing in-home care, day-centre, and community-based activities.
- Ensure all care services are delivered in accordance with client requirements.
- Maintain accurate documentation of client interactions and progress.
- Monitor client progress and adjust care plans as needed.
Tertiary qualification in a relevant discipline or equivalent experience.
Average financial and numerical literacy, including experience in managing budgets.
Excellent written and interpersonal communication skills.
Demonstrated problem-solving skills.
Proactive, flexible, and agile team player.
Honest, trustworthy, and reliable individual.
