We are seeking an experienced Dementia Care Specialist Lead to join our team at HammondCare Horsley.

Job Description

The Dementia Care Specialist Lead will be responsible for providing tailored care and support to individuals living with dementia in our complex behaviour cottages. This role requires a high level of clinical expertise, strong leadership skills, and the ability to work collaboratively with a wide range of stakeholders.

Key Responsibilities:

  • Advocate for residents to ensure their care is person-centred and focused on their strengths, capacities, and well-being.
  • Facilitate clinical care delivery by Registered Nurses and Carers.
  • Provide hands-on modelling, coaching, and mentoring in clinical and complex behaviour management areas and processes.
  • Consult on case management and clinical decision-making.
  • Oversee clinical care and ensure professional competence as a Registered Nurse with experience in complex dementia-specific care.
  • Support and investigate clinical/resident incidents as required.
Required Skills and Qualifications

To be successful in this role, you will need:

  • Tertiary qualifications in specialised dementia and/or mental health service nursing for older people.
  • Significant experience operating as a consultant or specialist nurse preferably within Aged Care.
  • Demonstrated ability to support complex behaviours and psychological symptoms of dementia, motivate and develop a team, and effectively work with other staff.
  • Strong leadership and influencing skills.
  • Attitude of service.
  • Strong negotiation and conflict resolution skills.
  • Excellent analytical skills.
  • Strong administrative and computer skills.
  • Ability to work independently and as part of a team.
  • Empathy and patience.
