About the role:

This is a non-clinical position that supports vulnerable senior members of our community to understand, access and connect with relevant resources in their area. The role sits within a support team and reports directly to a manager.

The Care Support Specialist works independently to engage in outreach, develop pathways for referrals, communicate with other service providers and offer support to clients. The goal is to identify barriers to services for vulnerable community members and facilitate access to these services.

The Care Support Specialist will act as a knowledge hub for community members, other service organisations and groups.

Key Responsibilities:

  • Engage in outreach and develop referral pathways
  • Communicate with other health/support services
  • Offer support to clients
  • Act as a knowledge hub for community members and other stakeholders

Requirements:

  • A minimum of 3 years' experience working in a community-based aged care or disability setting
  • Relevant qualifications such as Allied Health Assistant; Certificate III or IV Aged Care / Disability/ Leisure and Lifestyle
  • Excellent written and verbal communication skills
  • Proficient in using Microsoft Office suite of programs
