Aboriginal and Torres Strait Islander Transition Support Specialist

Job Overview:
The Aboriginal and Torres Strait Islander Transition Support Specialist will collaborate with young people from age 15 to successfully transition from care. The role focuses on developing skills and knowledge for independent living, delivering available funding, and ensuring support until the age of 25.
About the Role:
This position will work as part of the Care Team addressing complex trauma impacts on young people in out-of-home care and adults who have exited the system. You will use therapeutic crisis intervention techniques to assist behaviour de-escalation and develop comprehensive plans for each young person's transition to independent living.
Key Responsibilities:
- Work collaboratively with the Care Team to address complex trauma impacts on young people in care and adults who have exited the system.
- Utilise Therapeutic Crisis Intervention techniques to assist in behaviour de-escalation and help young people develop coping mechanisms.
- Develop comprehensive plans to support young people's transition to independent living and engagement with community supports.
- Work with Child Safety staff to support young people in care aged 15+ in developing a Transition and Financial plan.
- Facilitate opportunities for young people to participate in peer groups and networking to develop social support networks.
- Develop and maintain relationships with significant family members, friends, community organisations, and government agencies.
- Incorporate strategies within assessments and goal plans to maintain young people's culture and identity.
- Support and promote engagement with local Aboriginal and Torres Strait Islander communities and community groups.
