Case Manager - Transitional Housing & Rapid Rehousing

Job Overview

Under the supervision of the Young Adult & Rapid Rehousing department, and collaborating with Youth Homelessness Demonstration Program agencies, you will provide case management services to homeless and formerly homeless young adults The role utilizes the Transition in Place housing model to facilitate rapid exit from shelter, employing evidence-based practices such as Trauma-Informed Care, Motivational Interviewing, Harm Reduction, and Housing First.

Responsibilities
  • Develop and implement individualized service plans to help clients maintain housing and achieve personal goals, including mental health, substance use management, income increase, and self-sufficiency.
  • Conduct recertifications, home visits, and follow-ups to support housing stability and independence.
  • Promote a culture of diversity, equity, and inclusion through anti-racist practices across the department and agency.
Job Details

This is a non-exempt, union position with a starting wage of $20.00/hour. The work hours are Monday through Friday, 8:30 am – 5:00 pm.

Qualifications
  • Bachelor's degree in social work, counseling, or a related field.
  • Lived experience of homelessness or housing instability, especially among BIPOC, LGBTQIA+, and young adults in Cuyahoga County.
  • Experience in providing services to the target population and/or case management preferred.
  • Valid Ohio driver's license with less than six points and proof of automobile insurance, complying with Ohio's Financial Responsibility Act.
