Job Overview

As a Case Manager, you will empower individuals to live independently and safely in their community through our Linkages Program.

The program provides short-term, individualised case management and flexible support to help people achieve their goals.

About the Role

You will be responsible for empowering individuals through person-centred care planning and short-term support.

  • Empower individuals by identifying and addressing their unique needs and goals.
  • Build bridges between services, communities, and customers by providing coordination and advocacy services.
  • Champion inclusion by tailoring support to diverse needs and backgrounds.
  • Conduct comprehensive assessments and develop care plans that meet individual needs.
  • Coordinate services and manage Linkages brokerage funds effectively.
  • Collaborate with service providers and advocate for customer needs.
Required Skills and Qualifications

To be successful in this role, you will need:

  • Minimum 2 years' experience in case management.
  • Strong knowledge of services for younger people with disabilities.
  • Working knowledge of Home and Community Care Program for Younger People (HACC PYP).
  • Skilled in psychosocial assessments, care planning, and service coordination.
  • Tertiary qualification in Social Work, Community Services, or equivalent.
  • Current driver's licence and relevant checks (WWCC, NDIS screening).
Preferred Qualifications

Desirable qualifications include:

  • Experience with different government health funding streams.
  • Strong financial administration skills, including managing brokerage budgets.
  • Knowledge of wellbeing and reablement frameworks.
  • Familiarity with Carelink Plus client management systems software.
  • Experience working with people from diverse backgrounds or with low health literacy.
