**Job Opportunity:** Patient Flow Coordinator
We are seeking a highly skilled and experienced Registered Nurse or Allied Health Professional to join our team as a key player in patient discharge planning.
The successful candidate will work collaboratively with a multidisciplinary team to facilitate safe and timely discharge for patients in the acute hospital setting. They will be responsible for identifying patients who require assistance with their discharge, addressing any barriers to discharge, and escalating concerns to the relevant team members.
Key responsibilities include:
- Collaborating with the multidisciplinary team to identify patients who require assistance with their discharge
- Addressing or escalating any barriers to discharge
- Contribute to an improvement in access and flow KPIs
- Demonstrating a multi-disciplinary healthcare approach that achieves safe and timely discharge for patients
The ideal candidate will have:
- Current professional registration with the Australian Health Practitioner Regulation Agency or other allied health professional association
- A high level of clinical competence and expertise in a specified area
- Understanding of safe and timely discharge principles
- Demonstrated ability to work as part of a team and model interdisciplinary practice
