Discharge Planning Coordinator

Job Summary:

Our hospital is seeking a Discharge Planning Coordinator to lead the development and implementation of safe and effective discharge plans for patients in the acute care setting.

About the Role:

  • Responsible for maintaining a clinical caseload and overseeing the day-to-day operations of the Discharge Planning team.
  • Reports to the Manager of Access and Patient Flow.
  • Ensures adequate resources are allocated to areas of greatest need.
  • Liaises closely with key stakeholders across the hospital, including the Access team, Transit Lounge, ward nurses/ANUMs, and the Transport Manager.
  • Contributes to an improvement in the hospital's access and flow KPIs that include timely discharge, reduced elective surgery hospital-initiated postponements, and optimized patient flow.

About You:

  • Current professional registration with the relevant health practitioner regulatory agency or other allied health professional association.
  • High level of clinical competence and expertise in specified area.
  • Understanding of safe and timely discharge principles.
  • Ability to lead a team and model interdisciplinary practice.

Responsibilities:

  • In collaboration with the multidisciplinary team, identifies patients at risk of delayed discharge and develops strategies to address or escalate barriers.
  • Facilitates regular team meetings and upskilling of team members.
  • Maintains relationships with key stakeholders external to the hospital, such as transport providers, acute hospitals, and residential aged care facilities.
