Job Title: Occupational Therapist or Physiotherapist

Facilitate Patient Discharge

The Response Assessment and Discharge (RAD) Team is seeking an Occupational Therapist, Physiotherapist, or Nurse to join our team on a part-time basis. As a member of the wider Emergency Department (ED) team, you will work with patients to facilitate their safe discharge from the ED.

About the Role

You will be working in a trans-disciplinary model, providing assessment and discharge care coordination for patients in the ED. Your role will involve conducting patient assessments and interventions according to your scope of practice as an Occupational Therapist, Physiotherapist, or Nurse. You will report daily to the Nurse Unit Managers (NUMs) of the ED and then to the Operations Director of the ED.

Your Key Responsibilities

  • Conduct patient assessments and develop individualized plans to facilitate safe discharge
  • Collaborate with other healthcare professionals to ensure comprehensive care
  • Communicate effectively with patients, families, and healthcare teams
  • Document patient information accurately and efficiently

Requirements

To be successful in this role, you will need:

  • Current registration as an Occupational Therapist, Physiotherapist, or Nurse through the Australian Health Practitioner Regulation Agency
  • Excellent communication and interpersonal skills
  • A strong ability to work effectively in a fast-paced environment
