Community Care Specialist

We are seeking a highly motivated professional to provide holistic care and support to our clients in their community.

  • This role involves working closely with clients, undertaking assessments and care planning to meet their identified needs.
  • You will be responsible for providing oversight on the care needs of our clients, liaising with staff, health and service providers as necessary.
Key Responsibilities:
  • Providing clinical assessment and care planning services to clients
  • Liaising with healthcare workers and enrolled nurses to ensure effective care delivery
  • Ability to prioritize and manage workloads according to client needs
  • High-level communication, interpersonal, and negotiation skills
  • Excellent computer literacy skills including Microsoft Office
