About the Role

We are seeking a highly skilled and experienced professional to lead our Social Support Services team at Castlemaine Health. As a key member of our organization, you will be responsible for planning, developing, and delivering programs aimed at promoting physical, emotional, and social well-being.

Key Responsibilities
  • Lead the planning and development of social support programs, ensuring they meet the needs of our clients and stakeholders.
  • Collaborate with colleagues and external partners to achieve organizational goals and objectives.
  • Develop and maintain relationships with clients, families, carers, and stakeholders to ensure effective service delivery.
  • Monitor and evaluate program effectiveness, making recommendations for improvement.
  • Ensure compliance with quality standards, policies, and procedures.
Requirements
  • Proven leadership skills with the ability to inspire and motivate team members.
  • Experience in planning, delivering, and evaluating social support programs, preferably within the healthcare sector.
  • Knowledge of CHSP, HACC-PYP, and NDIS frameworks and funding requirements.
  • Strong interpersonal and communication skills, with the ability to engage effectively with clients, families, carers, and stakeholders.
  • A person-centred care approach, dedicated to meeting the individual needs and interests of clients.
  • Demonstrated ability to maintain compliance with quality standards, policies, and procedures.
