Key Responsibilities:- Develop and implement case management plans tailored to meet individual needs and goals.
- Provide support services including crisis budgeting, information, advice, community referrals, and advocacy.
- Conduct initial and ongoing assessments of participant needs, including risk assessments and safety planning as necessary.
- Deliver respectful, proactive, persistent, reliable, practical, and comprehensive support that maintains the dignity and privacy of individuals.
- Foster participants' capacity for independent living by maximising their skills and resources.
- Support participants in accessing housing and specialist services such as drug or alcohol treatment, mental health support, legal assistance, domestic and family violence services.
- Certificate IV qualifications in Community Services or similar fields.
