Job Title: Community Mental Health Practitioner

We are seeking a resilient and compassionate professional to provide intensive, person-centred support to individuals experiencing mental health challenges.

  • Support people to transition from hospital to community living.
  • Engage clients using therapeutic frameworks and practices including risk mitigation, trauma-informed care, and family-inclusive practice.
  • Plan and deliver group programs to support recovery and skill-building.
  • Support families and carers with education, information, and referrals.
  • Collaborate with community services to ensure coordinated and integrated support.
Key Responsibilities:
  1. To work with clients to develop strategies for independent living.
  2. To engage in activities that promote social inclusion and community participation.
  3. To assist clients with daily residential operations, including housekeeping duties.
Requirements:
  • Tertiary qualifications in Mental Health, Psychology, Social Work, Occupational Therapy or other health-related field.
  • Current First Aid and CPR certificates.
  • Experience working with people with mental health challenges and complex needs.
