We are seeking a highly skilled and compassionate professional to join our dynamic team at Western Sydney Local Health District (WSLHD). As a Senior Intellectual Disability Health Service Social Work Professional, you will play a crucial role in enhancing health outcomes for individuals with intellectual disabilities and complex health needs.

About the Role

This is an exciting opportunity to work in a multidisciplinary outpatient service that is committed to innovation and excellence. You will be part of a team that includes social workers, clinical psychologists, psychiatrists, nurses, and medical professionals who share your passion for delivering high-quality care.

As a key member of our team, you will be responsible for delivering time-limited, client-centred services, including assessment, care planning, and navigation for young people and adults with intellectual disabilities. You will also work closely with primary healthcare providers and mainstream services to enhance their capacity to meet the needs of this population.

In addition to your clinical expertise, you will have excellent communication and interpersonal skills, with the ability to build strong relationships with consumers, families, and carers. You will also be able to initiate, lead, complete, and share quality improvement initiatives and services evaluation processes.
