Specialized Clinical Care Coordinator Role Overview

Join a dynamic team providing high-quality care services to detainees with complex needs.

The Disability and Complex Care Coordinator plays a critical role in the Offender Reintegration program, supporting detainees with disabilities and complex behaviors. The successful applicant will be responsible for coordinating specialized clinical care and providing advice on disability and complex needs to inform positive behavior support plans.

Key Responsibilities:

  • Deliver multidimensional treatment options for detainees with varied and complex needs
  • Provide advice on care and service provision for detainees with disabilities and complex needs
  • Contribute to accommodation unit placements and regular care coordination

Requirements:

  • Demonstrated experience in providing assessment and interventions for detainees with complex psychosocial and/or physical needs
  • High level interpersonal, negotiation and communication skills
  • Australian citizen, permanent resident or valid work visa holder
  • Current driver's license
  • Registered Nurse - Mandatory: hold registration with Australian Health Practitioners Regulation Agency [AHPRA]
  • Occupational Therapist - Mandatory: hold general registration with the Occupational Therapy Board of Australia (Australian Health Practitioners Regulation Agency [AHPRA])
