Key Responsibilities- Conduct medication chart reviews and medication reconciliation
- Dispensing and supplying medication and medication profiles
- Coordinating patient medication requirements for inpatient stay, day infusions and discharge
- Developing and maintaining professional relationships with key stakeholders to ensure excellent customer service to clients and patients
- Supporting patients, carers, hospital staff and associated employees
- Educating and advising staff and customers on health promotion, disease prevention, drug selection and the proper use of medicines
- Be a current AHPRA registered pharmacist or a Pharmacy intern awaiting general registration from AHPRA.
- Dispensing experience in an Australian based hospital or community pharmacy
- Current drivers licence and roadworthy NSW-registered vehicle as role requires travel to various sites within our NSW pharmacy network.
- Strong knowledge of the Pharmaceutical Benefits Scheme (PBS).
- Value attention to detail and able to work autonomously.
