Job Title: Home Care Specialist

In this unique role, you will have the opportunity to build meaningful relationships with our residents and create a supportive environment that encourages independence and personal identity.

The day-to-day rhythm is defined by the residents, allowing for flexibility and variety in your work. You will collaborate with our Health and Wellness Team and Dementia Support Consultant to provide high-quality care and support our residents in living their best lives.

As a Home Care Specialist, you will:

  • Develop and maintain strong relationships with residents and their families
  • Provide individualized support and care to residents in their homes
  • Work collaboratively with the Health and Wellness Team and Dementia Support Consultant to ensure high-quality care and support
  • Participate in ongoing training and professional development to enhance skills and knowledge
  • Contribute to the development and implementation of care plans that meet the unique needs of each resident

To be successful in this role, you will require:

  • A Certificate III in Individual Support or Aged Care
  • Experience working with aged people living with dementia or in similar settings
  • Strong interpersonal and communication skills
  • Ability to work independently and as part of a team
  • Flexibility and adaptability in a fast-paced environment
