We are seeking a dedicated professional to enhance the lives of our residents by providing individualized support and enabling them to continue their activities and lifestyle choices.

This includes fostering meaningful relationships with friends and family, engaging in local groups or pursuits such as gardening, painting, or singing. To achieve this, the successful candidate will take a genuine interest in getting to know our residents as individuals, creating life history books, and sharing memories.

The ideal candidate will promote social and recreational aspects of daily living by collaborating with the local community and its resources.

  • Experience working with vulnerable adults is essential.
  • The successful candidate must be IT literate and able to compile comprehensive social care plans for residents.
  • They will possess excellent interpersonal skills, effective verbal and written communication skills, and the ability to maintain confidentiality.
  • Priority will be given to candidates who can work independently and as part of a team, demonstrate a keen willingness to learn and improve performance, and have a proactive attitude.

This role requires creativity, innovation, and enthusiasm to ensure residents lead fulfilling lives.

Key Responsibilities:
  • Foster positive relationships with residents and understand their unique needs and preferences.
  • Create personalized life history books and share memories with residents and their families.
  • Promote social and recreational activities within the home and engage with the local community to enrich the lives of our residents.
  • Compile comprehensive social care plans for residents and communicate effectively with them, their families, and healthcare professionals.
  • Maintain accurate records and confidential information.
  • Work collaboratively with colleagues to provide an exceptional service.
