Population Health Specialist

This role is responsible for leading and coordinating work to consolidate, progress, and report on population health initiatives across the Grampians region.
The successful candidate will have strong communication and project management skills, as well as experience in leadership roles within health promotion, disease prevention, or population health.
Key Responsibilities:- Coordinate population health streams to achieve specific goals and objectives.
- Develop and maintain cohesive working relationships with key stakeholders and partners to effect planning, delivery, and timely evaluation of population health initiatives.
- Map and monitor local evidence-driven and/or Department of Health-priority population health streams.
The ideal candidate will possess a tertiary qualification in Population and Public Health or a relevant stream field, sound knowledge of public and population health risk factors, demonstrated success in leadership, recognised decision-making skills, excellent communication skills, and experience in initiating and managing quality relationships.
Required Skills and Qualifications:- A tertiary qualification in Population and Public Health or a relevant stream field.
- Sound knowledge of public and population health risk factors and their social determinants drivers in the Grampians region.
- Demonstrated success in leadership in health promotion, disease prevention, or population health (or a related endeavour).
- Recognised decision-making skills to solve unforeseen problems which threaten program outcomes.
