Job Description

About our Supported Lifestyle Lead role:

This is an exciting opportunity to lead and deliver a supported lifestyle program for seniors, catering to diverse needs including those living with dementia.

The successful candidate will be responsible for creating a constructive and supportive channel of communication with residents and their family members, developing a Lifestyle Assessment Plan that reflects the resident's interests and capabilities.

  • Foster strong relationships with residents and families to understand their unique needs and preferences.
  • Plan group activities to improve program attendance and create a sense of belonging among residents.
  • Identify opportunities to integrate new residents into the program, enhancing its footprint within the village.
Key Responsibilities
  • Create and implement individual Lifestyle Care plans, ensuring a holistic approach to addressing residents' social, emotional, and cultural well-being.
  • Guide the Supported Lifestyle team on Montessori methods for delivering these plans, promoting independence and empowerment.
  • Develop a range of activity options, both internal and external, to engage residents within an agreed-upon budget.
  • Promote the Lifestyle program among staff and stakeholders, highlighting its value and impact.
  • Maintain positive relationships with the Care team, using village platforms to record resident progress and engagement.
  • Collaborate with village operations to ensure a comprehensive approach to addressing residents' needs.
