Job Title: Coordinator – Healthy Hub Connect Program

About the Role:

We are seeking a highly skilled and experienced Coordinator to lead the Healthy Hubs Connect (HHC) model of care from our Coffs Harbour office. As a key member of our team, you will play a crucial role in implementing and overseeing processes at the hub to promote collaborative care for service users.

Key Responsibilities:

  • Engage directly with clients in a brief capacity, as well as oversee their care journey both internally and externally to the relevant hub.
  • Implement and oversee processes at the hub to promote collaborative care for service users.
  • Co-implement a model of brief intervention after-care for targeted HHC customers.
  • Monitor outcomes and demand associated with the HHC model of care, and collaborate with HHC partners to coordinate impactful collaborative responses to local needs.
  • Work in partnership with the local HHC lived experience workforce to build community-based responses at the Hub.

Qualifications and Experience:

We are looking for someone with extensive experience working within a mental health setting supporting young people and families, and a commitment to delivering a person-centred approach. An ability to work collaboratively and professionally with other services and professionals across a range of life areas and needs, as well as a knowledge of the mental health sector and services offered to the community to support those with complex needs will be essential to succeed in this role.
