Care Coordinator

The Team Leader plays a pivotal role in ensuring the delivery of high-quality care to our residents at all times. This involves supporting the Clinical Care Coordinator and Registered Nurse to supervise and coordinate care across the home, while actively ensuring the individual needs, wishes, and goals of each resident are met in a supportive and respectful environment.
Key Responsibilities- Provide emotional support to residents, ensuring care provision is holistic and person-centered.
- Support the coordination of care needs across the home, including medication management, nutrition, and hydration, and pressure care management.
- Lead and supervise care on the shift, within designated wings or areas.
- Perform medicine-related tasks in accordance with scope of qualification, state or territory legislation, regulation, and Calvary policies and procedures.
- Relevant qualifications: Diploma of Enrolled Nursing (with APRHA Registration to administer medication), Certificate IV in Ageing Support (including HLTHPS007 Administer & Monitor Medicines), or Certificate III in Individual Support with willingness to undertake training to obtain required qualifications.
- High level of commitment, integrity, honesty, and trustworthiness.
- Effective listener with ability to communicate at all levels, both orally and in writing.
- Ability to understand and use information technology and programs, and to learn new skills as required.
- Current National police check.
- Working knowledge of WH&S and Infection Control.
