Nurse Practitioner Position

We are seeking a highly skilled and experienced Nurse Practitioner to join our team. This is an autonomous yet collaborative role, providing expert clinical assessments and implementing advanced care plans across residential aged care, hospital in the home, and community settings.

About the Role

  • Provide high-quality patient care through expert clinical assessments and advanced care planning.
  • Lead initiatives in evidence-based practice and service development.

  • Collaborate with interdisciplinary teams to enhance patient outcomes across care environments.

Requirements

  • A registered nurse with current registration and endorsement as a Nurse Practitioner or working towards same.
  • Advanced skills in diagnostics, assessment, referral, and treatment planning.
  • Strong background in palliative care models and interdisciplinary teamwork.
  • Leadership capabilities with a focus on education, mentorship, and service development.
