About The Role

The Clinical Care Coordinator plays a vital role in ensuring the highest standard of care for our residents. Reporting to the Director of Care, this position oversees the personal and clinical care services within our residential aged care facility.

Key Responsibilities
  • Co-ordination of Clinical Care: Coordinate the clinical and healthcare needs of all residents, ensuring their care needs are met and regularly reviewed.
  • Care Planning: Assist with the development and maintenance of clinical documentation, assessment, and care planning for all residents.
  • Leadership and Guidance: Provide direction, advice, and supervision to care and clinical staff regarding individual care needs.
  • Quality and Compliance: Oversee the Medication Management Policy and ensure compliance requirements are met.
  • Communication and Teamwork: Maintain effective communication and morale between staff, management, allied, ancillary staff, and medical practitioners.
