Aged Care Clinical Coordinator Lead

Job OverviewWe are seeking an experienced Registered Nurse to fill the position of Aged Care Clinical Coordinator Lead. This role will oversee the clinical care and services within our residential aged care facility, ensuring a high standard of quality and safety for our residents.
Key Responsibilities:- Coordinate the clinical/health care needs for all residents, ensuring that their care needs are met and regularly reviewed.
- Assist with the development and maintenance of clinical documentation, assessment, and care planning for all residents.
- Provide direction, advice, and supervision to the care and clinical staff regarding individual care needs.
- Support and guide the RN In Charge in overseeing the care delivery team and ensuring resident wellbeing.
- Oversee the Medication Management Policy, including orders, administration, pharmacy follow-up, and advice to care and clinical staff.
- Coordinate with the Education Coordinator to ensure competency assessments are completed every 12 months minimum.
- Work with the Quality Coordinator to ensure compliance requirements are met.
- Maintain good communication and morale between staff, management, allied health professionals, and residents' families.
- Conduct case conferences and multi-disciplinary team meetings to review and evaluate resident care.
- Registered Nurse with current registration with the Australian Health Practitioner Regulation Agency (AHPRA).
- Demonstrated experience in residential aged care nursing, particularly in clinical assessments, care planning, clinical indicator analysis, and medication management.
- Ability to operate independently and professionally with limited supervision.
- Effective communication and teamwork skills, with the ability to work in a multi-skilled environment.
- Highly developed organisational skills and ability to determine work priorities.
- Understanding of the ageing process and commitment to advocating for older people in residential care.
- Commitment to ongoing professional development.
- A competitive salary of $120,000 - $150,000 per annum, depending on experience.
